Step 3: Liability Waiver

DRIFTLESS DASH 5K - LIABILITY WAIVER AND RELEASE

PLEASE READ CAREFULLY BEFORE SIGNING

I understand that participating in the Driftless Dash 5K on May 29, 2026, is a potentially hazardous activity. I should not enter and participate unless I am medically able and properly trained. I agree to abide by any decision of a race official relative to my ability to safely complete the run/walk.

ASSUMPTION OF RISK: I assume all risks associated with participating in this event including, but not limited to: falls, contact with other participants, the effects of weather, traffic, and course conditions, and all other risks associated with a public road race. Having read this waiver and knowing these facts, and in consideration of your acceptance of my entry, I, for myself and anyone entitled to act on my behalf, waive and release the Driftless Dash 5K, La Crescent-Hokah School District, Basketball Booster Club, the event organizers, volunteers, sponsors, and all municipalities and their agents from any and all claims or liabilities of any kind arising out of my participation in this event.

INHERENT RISKS: I understand that the risks inherent in this recreational running/walking activity include, but are not limited to: physical exertion, uneven surfaces, weather conditions, interaction with other participants, and the possibility of injury.

MINNESOTA LAW NOTICE: Pursuant to Minnesota Statute § 604.055, nothing in this agreement purports or intends to waive liability for damages, injuries, or death resulting from conduct that constitutes more than ordinary negligence. This waiver applies only to inherent risks of the activity and ordinary negligence.

MEDICAL TREATMENT: I grant permission to event officials to authorize emergency medical treatment if necessary. I assume responsibility for all medical expenses incurred as a result of training for and participating in this event.

PHOTO/VIDEO RELEASE: I grant permission to the Driftless Dash 5K and its partners to use my photographs, videos, or other records of this event for promotional purposes without compensation.

ELECTRONIC SIGNATURE ACKNOWLEDGMENT: I understand that by typing my name below and checking the agreement box, I am providing my legally binding electronic signature in accordance with the Minnesota Uniform Electronic Transactions Act (UETA). This electronic signature has the same legal effect as a handwritten signature.
